Talk to your veterinarian about possible health issues. Urinary infections, diabetes, kidney failure and hyperthyroidism can all lead to litter box accidents.
Consider what has changed in the cat's environment since the cat started the behavior. Litter box problems can be caused by stress. Try to change whatever it is back to the way it was before the problem started. For example, if you changed brands of cat litter, go back to the brand you were using.
Clean the litter box more frequently. Cats don't like using a dirty litter box, and they have a sense of smell that is at least 20 times better than a human's. Scoop the soiled litter every day, or several times a day if you have multiple cats.
Move the litter boxes to more secluded locations in the house. Cats prefer privacy when using the bathroom.
Add more litter boxes. At minimum, you should have one box for each cat. If a cat goes to visit the box and finds it already occupied, give it the choice of using a different box. If you have a multiple story house, have at least one box on each floor.
Prevent strange cats from accessing your yard or places where your cat can see or smell them. Cats are territorial animals, and seeing a rival cat can trigger urine marking behavior.
Eliminate the smell of urine from spots where your cat has urinated previously. If the cat smells urine, it will urinate in the same spot again. You can purchase enzyme based pet urine cleaners that will get rid of the smell. Follow the directions on the product package for best results.

Cats spray urine for a number of reasons, including to mark their territory, because of health problems, or just because they're stressed out. A cat will continue to spray urine until the underlying problem that caused the behavior is resolved. So it's just as important to understand why your cat is spraying as it is to clean up the aftermath. By looking at the situation from the cat's point of view and resolving the underlying issue, you can restore harmony to your home.
