Schedule an appointment with your veterinarian. It is important to have your cat checked to rule out any physical problems that could be causing the cat to go to the bathroom outside the litter box. There are many conditions that could be causing the problem, such as blocked anal glands, bladder infection, incontinence, arthritis and old age. If the problem is not physical, you will need to explore other options.
Provide a location for the litter box that is readily available but private. A bathroom, utility room or basement area are good choices. Make sure doors are left open to the area.
Keep the litter box clean. Cats are naturally clean animals. Clean the litter box twice daily and scrub it at least once a week. If you have multiple cats, this will need to be done more often.
Switch brands of kitty litter if your cat is going to the bathroom outside the litter box. Choices consist of clay, sand, paper, pine shavings and gravel. Each cat has a different preference. If you are using scented litter, try switching to unscented. The scent is mainly meant for the benefit of the cat's owner, and some cats will not use scented litter.
Don't allow any litter to remain on the floor outside the litter box. It confuses the cat into thinking it is part of his bathroom area.
Change the style of the litter box. There are open litter boxes and ones with domed lids. Some cats prefer the privacy of the lid.
Establish a regular feeding schedule for your cat. Try placing him into the litter box upon completion of a meal.
Remove any clothing, towels and papers from the litter box area. If your cat has begun to urinate or defecate on a towel or some item that has been tossed near the litter box, try cutting a small piece and placing it into the box. Some cats prefer to use soft items as their bathroom.

The reason can be the result of a physical problem ,but usually it is caused by a behavioral issue. Cats are natural and quick learners of litter box usage. When your cat goes to the bathroom outside of the litter box, action must be taken quickly to lessen the chance of it becoming a permanent habit.
