Before a microchip can be registered, you must first go to your veterinarian to have the microchip implanted. Call your veterinarian to schedule an appointment and to confirm that they offer microchipping. There is more than one company that provides microchips for dogs; Home Again and Avid are two well-known companies. Your veterinarian can help you in deciding which microchip is right for you. Implantation is easy and painless, with the microchip being gently inserted underneath your dog's skin.
After your dog has been given his microchip, you will need to get the application form for registration. Your veterinarian who microchipped your dog will provide you with the appropriate form needed.
Completely fill out the registration form. Make sure to print your information clearly and legibly so that your dog's microchip will be registered to you properly. Try to provide as much contact information as possible, including home and cell phone numbers, as these are the methods that the company will use to try to reach you if your pet ever goes missing.
Include a check or your credit card information to pay for registering the microchip. Depending on the company, they might require a one-time fee only or a yearly fee.
Address a letter to the appropriate contact listed on the application. Provide the proper postage and mail the form out.
