Visit the websites of reputable service dog breeders in America. For example "Assistance Dogs International" and "Wolf Packs" and "Great Plains Dogs". All of these websites contain locations of accredited service dog providers all over the country.
Submit an application to the program nearest you. You will have to include documents of proof of your disability. For example, a doctor's recommendation and physical evaluation are typical.
Write down information about your lifestyle and habits and describe a typical day. The application will ask about your everyday life where you can show how a dog can assist you.
Provide personal references. The application might ask for recommendation letters from various people in your life, such as a teacher, friend, and doctor.
Include a photograph of yourself, so that the service dog providers can match you with an appropriately sized dog.
Submit your application to the accredited program and wait for approval. Once your application is approved, you may need to visit the center to undergo training with your dog.
